Please note that international pricing is based on smaller items and is subsidized as we send the DVD's via a Royal Mail tracked and signed for service.
Where the weight of an order may become a factor in the subsidized shipping cost (Mainly relevant to hard copy printed forms) we will confirm the shipping costs for each order before shipping.

The collection download option contains our entire clinical assessment range. This options saves you shipping costs and provides you with a download link. We will send you a secure download link to the files once we process your order.

Every client you see should have a comprehensive medical assessment as part of their initial consultation and treatment plan. Our forms take away the headache of making up or photocopying your old, outdated assessments.

Present your practice or place of work in a professional & friendly way whilst maintaining your duty of patient care. The download contains files that can be used in two ways:
- Full colour desktop PDF versions for printing as you need them from any desktop printer, and
- Full colour PDF versions for printing in bulk from your local professional printer. This allows you to print at a reduced cost in your local country. All forms come with additional printing notes for the printers.
This is a single license product that you are buying and so can only use the images for your specific needs. Additional licenses should be bought for friends or colleagues wishing to use these questionnaires.

There are no return options for opened DVD's or CD's. Goods damaged during transportation will be replaced at our expense.
Unopened goods may be returned however must arrive in mint salable condition. The cost of returning goods is at the customers expense.
In relation to returned goods we will refund the original cost of the goods less the original shipping costs which cannot and will not be refunded.
